Tom Finn, NP
Hello, my name is Tom Finn, and I am a board-certified Nurse Practitioner specializing in podiatry. With over six years of experience in the healthcare industry, I’m passionate about maintaining healthy feet. My approach as a foot care provider is to treat conditions and educate patients on the significance of healthy feet and ways to achieve them. Originally from West Michigan, I received my doctorate in nursing practice from Grand Valley State University. Currently, I reside in Grand Rapids, Michigan, with my wife and one-year-old son. During my leisure time, I enjoy exploring the outdoors and hiking with my family.
Miriana Popadich, NP
My name is Miriana Popadich, I am a board-certified Family Nurse Practitioner. I have over 25 years of experience working in the healthcare field and have been working as a Nurse Practitioner for over 13 years. I live in Monroe, Michigan, where I have lived for over 40 years. My passion is working with senior adults and providing them with primary care in their homes. As a certified Nurse Practitioner, I can prescribe and manage your medication and effectively manage all your primary health needs. I strive to visit you at home once per month or more if needed. I look forward to providing you with excellent care and being able to take care of your primary care needs.

I received my Masters of Science and Nurse Practitioner from Wright State University in 1998. I LOVE working at HomeMD! There is nothing better than seeing my patients in the comfort of their own home. I am then able to provide the individual care that works for not only the patient, but for their support system as well. I also have experience in pain management through my time working at Michigan Pain Consultants. I am a Wife & Proud Mom of 3 children and adoring Grandmother of two beautiful grandchildren. I enjoy relaxing at our family cabin up north and I love my Ohio State Buckeyes. “OH-IO”
Jill Geer, NP
Hi, my name is Jill Geer. I started my nursing career after graduating from Saginaw Valley State University with my Bachelor’s of Science in Nursing. As an RN I primarily worked in the ER at various hospitals, often moving around due to my husband’s military career. In 2012, I returned to school to obtain my Master’s Degree and graduated in 2014 from Maryville University where I obtained my nurse practitioner degree. I have spent several years working as a Nurse Practitioner in the primary care setting as well as urgent care. My family has recently settled in Mid-Michigan after my husband’s military retirement. Our children keep us busy but, in my downtime, I love camping, relaxing by lake, and watching football. I look forward to meeting you and providing you or loved one excellent care in the privacy of your home.
Lindsay Pizzuto, NP
Hi, my name is Lindsay Pizzuto. I began my career as a Home Care RN after
graduating from Calvin College in 2007. I earned my master’s degree from
Duquesne University in 2013 and began working at a Family Practice Clinic in Aiken, SC. I have now found the perfect mix of my favorite jobs with HomeMD. I think that we can more practically reach health goals when I can see patients in their homes and not have them sitting in waiting rooms. I believe every person has inherent worth, and I feel privileged to be invited into your home. Outside of work, I enjoy spending time outdoors with my family, working on our hobby farm, and serving with my church. I look forward to meeting you!
Aaron Collins, NP
Hello! My name is Aaron Collins, and I’m a nurse practitioner and a graduate of the University of Michigan. My primary focus is on ensuring the health and wellness of my patients. I work collaboratively with them and their families to establish a common goal for overall health. My areas of expertise include managing chronic diseases and treating rheumatologic conditions. When I’m not working, I spend time with my lovely wife and two boys, who enjoy playing with their trains and participating in sports. Tywanna Tanner, AGPCNP-BC
Hi, my name is Tywanna Tanner, and I am an Adult Gerontology Nurse Practitioner-BC and a Walden University graduate. Most of my background is in hospice, but I also specialize in podiatry. I enjoy working for Home MD because of the many programs they offer to the community. Working for HomeMD is like working with family, and I love that everyone is willing to assist each other and make patient care a top priority. When not caring for patients, I enjoy spending time with my family. I am married with 5 beautiful daughters and have a beautiful energetic 5-year-old granddaughter. I love to serve my community and be a blessing to others and I look forward to partnering with you in your care.
Amber Dortch, NP
Hi, my name is Amber Dortch. I graduated from Duke University and am a board[1] certified Family Nurse Practitioner. I have spent my career as a Nurse Practitioner so far, in primary care, pain management, and mental health. Prior to this, I worked in the emergency department for nearly 18 years as an RN. Outside of my clinical practice, you will find me writing nursing exam questions, and editing nursing textbooks. During my free time, I’m either skiing down a mountain, relaxing on my boat, or driving my kids to practices or games. I look forward to getting to know you and working with you towards your health goals.
Andrea Sylvester, NP
Hello! My name is Andrea Sylvester and I have been a nurse practitioner for the past 10 years. I attended Aquinas College for my nursing degree, and I knew Grand Rapids was the place where I wanted to settle down and raise my family. I have a loving husband and two children who love to keep us busy in our free time. I have a passion for the geriatric population, focusing on the patient’s quality of life! I am passionate about quality care and taking the time to make sure my patients understand their care plan. I believe it is important to build a strong relationship; not only being reactive to urgent health care concerns that may arise but also preventative with frequent contact and monthly appointments. Hi, my name is Deborah Yeldo. I have been serving patients in the community since 2013. I received my B.S.N from Wayne State University and my M.S.N at Purdue University. I treat patients using shared decision-making and aim to provide comprehensive care promoting patient health and well-being. The thing I love about my job is providing care to patients in their homes. I have dedicated my days to serving patients and my evenings to spending time with family. When not caring for patients, I enjoy the change of seasons in Michigan and the variety of activities our state offers.

I live in Sunfield, MI. with my husband and two teenage boys. My passion is working in behavioral health and providing specialized care in the home or living environment. As a certified nurse practitioner, I can prescribe and manage your psychotropic medications, and effectively oversee all your behavioral health care needs. My patients will be seen once a month and/or sooner based on their individualized needs. In addition to my services, you will have the benefit of access to an after-hours Nurse Practitioner that can provide support to you, your community and loved ones when you need it the most. Hello, my name is Kelsey Crampton. I am an Adult Gerontology Nurse Practitioner who has been in the healthcare industry for over a decade. I am passionate about providing comprehensive, patient-centered, and holistic care to all my patients. My journey in healthcare began as a certified nurse aide in homes and progressed to an RN role at a hospital after obtaining my bachelor’s degree in nursing. I have recently finished my Doctorate in Nursing and, since then, have been working in primary care. I enjoy getting to know my patients and find fulfillment in my work. Born and raised in the Blue Water area, I have a deep love for my hometown and intend to make it my permanent residence for many years to come.

I am an Education Chair for the Ann Arbor Chapter for Michigan Council of Nurse Practitioners. Sara Sniff, NP
Hi, my name is Sara Sniff. I graduated from Davenport University in 2019 with my MSN with a concentration in (FNP) family nurse practitioner. I spent my first year as an FNP working in an advanced wound clinic which allowed me to become wound care certified as an NP. I then moved over to an internal medicine office to solidify my skills as an FNP to become a well-rounded provider. Before obtaining my Masters I worked as a CNA, a certified surgical technologist, and as an RN. I have worked in skilled facilities, assisted living, hospice, and now home health. I am passionate about providing excellent care to the underserved and hard to find. I am a resident of Northern Michigan, Traverse City. I am happily married to my husband of 30 years. We have one daughter who is 21 and 2 fur babies. When I am not caring for patients, I enjoy dirt biking, hunting, hiking, and camping with my family. Stacey Berlin, NP
Hello, my name is Stacey Berlin. I’m a nurse practitioner and an Air Force Veteran. I graduated from Purdue Global University in October of 2018 and have been working in primary, palliative, and urgent care settings ever since. I also hold a bachelor’s degree from Saginaw Valley University and have practiced nursing in multiple specialties between 2009 and 2018. Prior to becoming a nurse, I was a physical therapy assistant and obtained my associate’s degree from the community college of the Air Force. Apart from caring for patients, I love spending my free time weightlifting, doing CrossFit, and occasionally competing in local competitions. I cherish my family and have been in a committed relationship with my boyfriend for 13 years and have a total of three kids (two of whom are grown) and two dogs. I look forward to working together and helping you achieve your health goals.
